Our Model

We begin by cultivating an interagency collaborative that serves as a network of pro-social supports along the continuum of care model. Partnering agencies provide evidence-based programs and services to students at risk of poor life outcomes.

Our continuum of care model is based on the premise that the complex needs of Clayton County’s most vulnerable youth can best be addressed through the coordinated care of multiple agencies and committed actors in the county.

We work with parents and guardians to alleviate the barriers that hinder their child’s participation.

Parental and guardian input is valuable and encouraged throughout the program.

System of Care Process

Moving Towards Healing


Clayton County Public Schools identify students who are in need of Clayton County System of Care Services.


Eligibility Criteria
Students who meet one or more of the following criteria are eligible for referral to Clayton County System of Care (CCSC) Staffing/intake Department.

  • Discipline Referrals
  • Out of School Suspensions
  • School Based Probation
  • Exposure to Trauma
  • Internalizing Behaviors
  • Safety Assessment
  • Failing Courses/Lack of Credits


The parent or guardian will be contacted to attend a meeting with the CCSC panel and the school to discuss their child’s strengths and needs.

Based on the student’s needs, the parent is offered one or more of the following services. All services are provided by CCSC certified providers.

  • Community Support
  • Individual Therapy
  • Group Therapy
  • Family Therapy
  • Intensive Individual Mentoring
  • Intensive Mentoring
  • Job Readiness/Workforce Development
  • Trauma Responsive Care
